Medication list for a hospital visit: what to bring
The first thing a hospital asks, planned or not, is what the person takes. A current printed list answers it in a minute and spares everyone from reading bottles in a corridor.
What the list needs
- The person's full name and date of birth.
- Each medicine's name and strength, exactly as on the label, including anything bought over the counter, inhalers, drops, creams and supplements.
- How it is taken, as prescribed: when, and how often.
- The prescriber for each, and the pharmacy.
- Allergies and reactions, written as you were told them.
- The date you printed it, so the reader knows how current it is.
Keep it ready
- Update it the day anything changes, not the day you need it.
- One copy in the bag you would grab for an emergency, one on the phone.
- Bring the bottles too if you can; the list is what gets read first.
